Dear Member,

On behalf of the European Society for Philosophy of Medicine and Health Care I invite you to send in your membership fee for the year 2008, before the end of July 2008.

The fee is €40, - (forty euros) for 2008

This request can be met by sending your fee either:
1. Through regular bank channels (thus not by cheque) in euros to the Society's account number 3701.08.892 at the Rabo Bank, Kerkstraat 1, 1551 BN Westzaan, the Netherlands.

2. Through the postal giro at the number 5780894, ESPMH, Treasury, Kerkbuurt 11, 1551 AB Westzaan, the Netherlands. Other countries than the Netherlands, add to your fee the giro handling costs of € 3,50
3. Bank cheque to the ESPMH, Treasury, c/o Kerkbuurt 11, 1551 AB Westzaan, the Netherlands, add the Bank's handling costs of regular cheques of € 10,-.
4. Through American Express/Master Card/Visa.
Mail/Send your name, address, card number and date of expiration to the Treasury. There is one important additional request: send in also the last three digits, in case of Amex: four digits, at the back of the card or the security code on the front, in order to complete payment. Without these digits the collection process cannot be completed. Send these digits in a separate email.
It is clear that a preference is expressed for route 1, 2 and 4, being the ones that involve little or no extra costs and the easier route for some international members.

PS Members who have not paid before the date of September 1,2008, will be taken off the membership list, while their obligation to pay does not cease. Members with a backlog in payment run the risk of receiving separate bills for past years of receiving the European Journal.
